Blood Tests

Seems like when you go to order a package deal for blood work they always exclude one important blood test.

Ideally I would like a Blood Test Package that includes:

Estradiol
Follicle-Stimulating Hormone (FSH)
Luteinizing Hormone (LH)
Sex Hormone binding Globulin, Serum
Testosterone , Free (Direct) With Total Testosterone
Thyroxine (T4)
Thyroid-Stimulating Hormone (TSH)
Tri-iodothyronine (T3) Total
Tri-iodothyronine (T3), Free, Serum
Dehydroepiandrosterone (DHEA) Sulfate
Insulin Growth Factor (IGF-1)

Anyone know of a place that has a package that includes all of the above blood draws?

(Ideally in the $200 Range)
