    I don't know much about gangs or mafia's, except for what I saw in Mickey Blue Eyes (LOL, that was a hilarious movie!!).

    Anywho, the question at hand isn't really how hardcore or ruthless they are, the question is, biggest and most organized, and as a result, i'm gonna have to go with Police and other Government Agencies. They are so big, they are so spread out across the world, and they are ALL linked together in one way, CIA and FBI and NSA, you can practically call it all one bit unit. And has a result, they would be the biggest.

    And that, is why I'm gonna go be a cop, and become a dirty dirty cop
