Is he/she quite old?
Like people, animals start to take care of themselves less as they get older. OR it might just be a alergy or mites/fleas. Vet time I think.

Put it outside and let it take care of it naturally. That is where cats and dogs belong.
If you wash them to much their skin gets dry.
