How do drug users hit a vien

As we all know when pinning during a normal cycle we avoid hitting viens. When i hit one i dont even have to aspirate because it hurts so bad and i just back right out. Now, i am clueless when it comes to recreational drugs outside aas. So how do drug users deal with catching a vien every time? Isnt it painful and bloody? Do they still get high if they dont hit it? I know its horrible but i am curious as hell.
