Where to start for Male Infertility?
So, I've been on TRT now for 7 or 8 years. I got divorced a few years ago and my new fiance (just turned 28) and I want to start a family. She had an IUD for the past 8 or so years and took it out 3 months ago. Since then we haven't been able to get pregnant. I took an at home Fertility test and it came up negative. I know this is just a basic test and not a true semen analysis. About 6 weeks ago I started taking 5000 ius' per week. I'm going to do another semen test to see if that's helped. I was planning on just adding in HMB to the mix and seeing what happens but I need to get a hip replacement in Feb and my fiance doesn't want to mess around with "bro" science and speculation.
That said, should I start with my primary doctor and schedule a Physical and bring up my concerns there to have them give me a referral or should I just find a fertility doctor on my own and then just get the referral that way? I'm thinking of coming off TRT as that's prob. my best option but was hoping to stay on if at all possible. I've dropped my dosage to 120mg/week but shut down is shut down.

You need to speak to an endocrinologist. Whether you go through your primary care doc or find one on your own is your choice. Probably easier if they know someone reputable.
HCG only replaces the LH. LH is responsible for stimulating Leydig cells to produce testosterone but not sperm. You really need HCG + HMG to be fertile... I am also curious what your doctor will say, because on the one hand you can drop everything and try it naturally, or you can supplement HCG + HMG - I'm not sure what would be better at this point. So keep us posted and good luck!

Come off the sauce, run a strong PCT & if all goes well you should be good within six months or so - if not less
If that don’t work - go from there

I’m torn about coming off or not. I have to have surgery in March and will have a 6 month recovery so now is the time if I come off. Just not sure how bad I will crash. I’ve lowered my dosage so I’m hoping it won’t be as extreme if I come off. I haven’t done a blast in over 7 months.
If I come off and run a solid PCT should I still take the HMG or just run a solid Clomid Nolva power PCT? I’ve only ever done one PCT ever as I went on TRT years after my first and only cycle I just remember feeling like death. Haha.

That was my exact Thought. It would make sense that the less you’re taking the easier the recovery from it would be. Getting your levels lower and coming off seems like it would be the way to go. If you’re giving at 1100 and drop to 200 you will feel like death but if you can go from 800 to 500 to 300 and then 200 shouldn’t be as dramatic

I’ve thought this out a few times already & have spoken to a friend of mine that did it. He was on TRT for over 5 year & juiced for a decade - it took them about 5 months to get pregnant
I say 3-12 months if all goes well
If you’re looking to just get er pregnant, set the eye on the prize & let the time fly - I’d expect to feel like royal shit & be working with a limp noodle till I’m back on - since I’ve suffered from low test way before I jumped on roids
I’d do something like, if you’re on 150mg weekly. Drop to 100 for a week or 2, then space the last one or two even farther apart - run HCG the whole time. Jump off, then run a strong PCT

I never stopped my TRT, i just added in HCG and all was good two months later. At the time we found out i had been on TRT for around 6 years.

You were able to get you woman pregnant in 2 months after taking just the HCG ? How much were you taking? I'm taking 5000ius per week and I'm on week 6 and nothing. I did a home fertility test which came up negative. Doesn't mean I don't have any swimmers just means I'm not at optimal levels.
I'm really really really on the fence about which route to go. I would prefer to just add the HMG and keep the HCG and get her pregnant if possible. But, if I were to just come off I'm guessing my chances would be greater. My goal is to get her pregnant by Feb 10th when I have my surgery but who knows if that's a reality. I'm really nervous about the crash if I come off but if I'm crashing and going thru surgery it won't matter as much.

My story is that I started lifting around 19 years old. I trained naturally until 33. I started at 125lbs. and my genetic max was 185-190. I did my first cycle 500mg/Test E 12 weeks and 4 weeks of PCT. I hit my goal of being over 200lbs. After that I just kept training and was able to keep my size so I was happy. When I was 35 I needed to have my hip replaced. I did that and took about 10 months off to recover. I went back to lifting and training BJJ and about a year from that I was always feeling tired and training and recovery was a challenge. I had my Test levels checked and it came back around 280 with normal being 250-1100. I started on TRT at around 37 years old. Started feeling AMAZING and then did a blast cruise once or twice per year since. When I went on TRT I was married with 1 son and we didn't want any more kids.
Fast forward, my wife sucked and I wanted out. I left her and found a young hottie. haha. She was only 24 and I was 41. We started dating and I got divorced etc. She didn't want kids at the time so all was good. We got engaged last year and she's since wanted to start a family. She decided in Aug to remove her IUD and start trying. We've basically been trying for about 2 months with no success. My first wife was pregnant the month we started trying. Now, with my surgery scheduled I would love to have her pregnant before then if possible.
I remember doing PCT and feeling like total dog shit. That is why I actually never did another cycle believe it or not. I never wanted to feel like that. Because of that feeling I'm reluctant to come off.

You can go & get your count checked. It is a possibility you can still have live swimmers - but, after years of being on it doesn’t sound likely.
My wife came off birth control almost 8 years ago & I have been apparently shooting blanks. We just said fuck it, and see what happens - I never came off - so far, straight nothing
Before the juice, she was knocked up within a month

This is exactly what I'm afraid of. Have you been taking HCG at all? I did contact a Fertility Clinic so hopefully they can give me some direction.

"Recovery of spermatogenesis in steroid suppressed patients
For healthy patients who use exogenous testosterone and are unable to establish a pregnancy because of the deficient spermatogenesis, there are now solutions to reverse the negative impact of testosterone supplementation. In our experience treatment involves discontinuation of exogenous testosterone and administration of 3000 units of hCG (either with the aromatase inhibitor anastrozole or the selective estrogen receptor modulator tamoxifen or clomiphene citrate) intramuscularly every other day for 3 or more months. As higher doses of hCG are known to suppress FSH levels, simultaneous administration of clomiphene citrate not only preserves, but enhances the secretion of FSH and LH from the anterior pituitary. With such treatments, testosterone-induced azoospermia was successfully reversed with hCG therapy in nearly all men receiving treatment. While further studies need to be carried out, every-other-day intramuscular hCG therapy is a viable option in the treatment of men who suffer suppressed spermatogenesis due to testosterone replacement . However, recovery is not immediate; patient spermatogenesis returned in 4–6 months."

Her IUD history shouldn’t be overlooked either. My wife had one for 10+ years. I had been on TRT for 4+ years as well and it took us near a year for our first son. Second one was no problem. I remember her OB stating that it would take her body a while to “normalize” as well...

Yes mate 2 months, i was doing 125iu's every 3rd day . I did start at 250iu's but my E2 went mad, Yes i know very little but it worked.
It was actually Kel that told me to reduce the HCG and gave me some information to read. Stressing yourself will not help mate, yes i know easier said than done.

I’ve decided I’m going to try coming off. I ordered the PCT meds and will lower my TRT dosage this week to 100mg and next week down to 80. I should have the PCT meds around then so I will come off and 2 weeks later start the PCT protocol with HCG . Since I have surgery coming up it makes most sense to just do it now. If I feel like crap it won’t matter as much as I will feel like crap post surgery.

Should I start taking the Nolva Clomid as soon as it comes in to start the jumpstart or should I wait 2 weeks after my last shot? Normally I would say to wait but since my goals are different should I start right away?

I’ve been down at 60mg per week now and def feeling it. Energy levels are way down and feeling sluggish. My strength is down about 10% in the gym. I typically do 400lbs on the Hammer Strength incline press and I’m doing about 360lbs. Could prob do more but lacking energy. I def feel tired all the time this past week. It could be something else going out but I doubt it. I took my final shot today and will start pct in 2 weeks from now

Im in a similar situation but a little worse. I actually got a vasectomy 15 years ago and have been on HRT for the last 10? New wife and we decided to try to have a kid. I thought the Endo would put me on hCG but instead he has me doing Clomid 50mg every other day and of course come off the Test. Ive read mixed results of off or on test so I decided to be safe.
We are not doing a reverse vasectomy but instead they will be going right to the source and removing it with a syringe. Yeah doesnt sound like fun. They will check for live swimmers and then freeze them and do in vitro fertilization. I will know if the clomid worked after January 17th.

Def. interested to see how you make up. Please keep me updated. Will they have to inject into your sack to draw it out? OUCH!!!!!!! lmao. Hopefully you have some swimmers and the Clomid works. I'm taking HCG at 2500 per week and Clomid. Running Clomid at 100mg/day for 2 weeks with Nolva at 40mg/day for 2 weeks then will drop to 50mg and 20mg/day.
Why does he have you taking the Clomid EOD? Did you do that once you came off?

I’m still on juice, haven’t started clomid & I’m still on my weekly HCG
Wife’s all for having another kid - out of all these loads, I say there’s quite possibly one that’ll still make it
Totally contradicting myself here - but, at this point in my life, I really don’t wanna drop the roids & go through the darn thing

I hear you. I was planning on just staying on and hoping the HCG would do the trick. So far nothing which is why I came off. I have surgery in 8 weeks so we are trying hard to get pregnant before then but who the hells knows. I’ve been on the HCG for almost 3 months now so those swimmers should start emerging. Haha.
My mind has been racing the past 2 days. I’ve been questioning decisions I’ve made in the past. I’m sure it is all just because my hormones are out of wack and I’m getting somewhat emotional. Def can’t continue thinking this way though. Haha
How long have you guys been trying?

Yup they extract with a needle going into the scrotum. Doesnt sound pleasant but I have had worse. Spermatocele removal. That wasn't much fun and I ripped a few of the stitches going out jet-skiing the next week. We dont get many 90 deg days in the summer around here so you have to take advantage of them. lol
Yes he had me stop (I tapered) off the test and he thinks the Clomid will work instead of the HCG. We will see soon enough, 1 more month.

But, what I just started to notice recently is that my nut seems more runny than we’d expect - it’s not a solid thick consistency
I been on so much juice & so long
Unfortunately, I might have to apply this to myself & do the tried & true way of coming off
But, same shit as u say - emotions start coming out when my hormones drop & get weird - it’s kinda y I just always stayed on

So, I'm at the emotional point hard right now. I'm 4 weeks since my last injection, 2 weeks on Nolva and Clomid. Starting to feel a bit better each day But nothing to get excited about. The EMOTIONAL part is kicking my ass right now. I keep looking back on my life and the decisions I've made and I'm second guessing them. thinking about my Ex which is scary as shit esp since she's mostly an evil person. haha. I'm def. more irritable and easily angered which has been the case in a very long time. I'm hoping when I drop the Clomid down to 50mg in a few days I will start feeling more myself. I'm guessing right about now is when my body should start the rebound process.
