If you're refering to "Is d-bol needed to kickstart a cycle?"
Answer is NO it is not NEEDED. You can use it though.

If you're trying to ask if Milk thistle is good? Yes you should start before you start the cycle and continue throughout PCT.

I suggest using Milk Thistle with any oral AS.

Milk thistle take a while to see any benefit. You should be taking off cycle at least 6 wks ahead of cycle (I take it every day). Stop taking during oral cycle, since silymarin has been shown to effect processing and absorbtion of orals. You can resume during Test E only portion and through PCT.

NAC is a better faster acting liver protectant.

How much milk thistle would you guys recommend? 1 gr a day? And is it better to split the dose throughtout the day or would a single dose suffice?

1g is sufficient. I take mine all at once.

imo, i think you should keep the dbol in with your test e. you would be happier with bulking. just my .02 from experience
