it does nither... you may have more blood flow for a lil bit in your penis but thats about it. therefore it may seem a lil thicker. now it doesn't necessarily shrink your nuts. it just makes em tighten up. just as if you were in cold water or just out of the pool. your sac and all just tightens up.
