abcess he can see a doctor for some anti biotics or wait till it goes down

If applying heat, 15-20 min should be enough, but you can do it several times a day if the area still hurts

cool i got a heating pad on right now on high
bump the heating pad. I'd have many a limp days had it not been for one of these fine creations. I personally only use the heating pad when needed. Sometimes when I shoot in my quads I'll hit a spot where it dosen't agree and I'll be sore for a few days afterwards. Delts and tris are two places where I've never had to use a heating pad.
you can also run the hot water on the site while you take a shower ....just aim the hot water (as hot as you can take it) right on the site and let the water hit it for a while ...feels good.
