Mixing injectables in syringe?
I've never done it myself, but I've come to understand that many do it to save time and (injection)space. Now, I've also understood that it is generally a totally ok thing to do, but just to be safe: which mixes are not ok/not recommended?

Mix away(anything that is administered meant via injection) ....don't mix anything that is not sterile.

.....you can mix oil and water based hormones in the same syringe as well.

Another thing:
Would it be fair to say that even if the injectables themselves are acceptably sterile, that the more times you change needles (which would generally translate to one extra time when mixing injectables), the higher the risk of bacterial contamination? Or is the increase in risk more or less insignificant? Of course, not changing needles at all would be much worse.

Its a good practice to change needle before you inject...as afr as loading i use the same syringe.
