Anyone use there gear when there was no caps on top

My Bro just got some gear and there are NO caps on top that you break off. One the the rubbers on top almost looks like a pin has already gone through it, what would be the cause of this?? He got it from a very decent source, he shot it anyway and I told him not to. Anyway why would this be like this?

sounds like it was used before, your bro will most likely get a bad soreness around the injection site, from not being sterile, Aids shouldnt transfer that way. Just throw the stuff out behind his back or something if he wont stop taking it, its' like shooting used crack.
