I had a similar thing and concluded it was just a minor infection... check how sterile your injection routine is... It seemed to me that I used the same needle for two seperate bottle to draw and usually I change everything out....also, wait for the alcohol to sterilize your injection area a few seconds after wiping.... i adjusted those things and didn't have a problem again... could be a multitude of variables imo....
