    Bloodwork

What should I ask them to check for as far as hormone levels are concerned?
also what kind of costs are associated with bloodwork?

Dont know about costs in the US but if you are starting a cycle you should get Liver Function, Testosterone both Free and Total, Estradiol and Cholesterol, if you are over 40 you should throw in PSA as well.

    Kale said it best!

In fact, I always get my blood tested without a doctor, and look over the results myself. No need for a Doctor, as most will just simply say "your tests are ok", and you'll never get to see the results unless you ask.
