I just fininshed my PCT about four weeks ago and im really tired. I want to know my test level. Is there a specific test i should be asking for when i call the doctor? And what reasoning can I tell them for why im looking to be tested?

Just tell him you think you're experiencing symptoms of low test, and you want to get your levels checked.
