If you hit a vein anywhere in the body, the first organ the oil will encounter is the lungs (where the blood is oxygenated, then sent to the heart and pumped out to the brain and body). The oil will coat a portion of your lungs, causing a coughing fit. Theoretically, the oil could cause a pulmonary embolism (potentially fatal), but I doubt you could accidentally inject the volume needed for this. I've never heard of anyone actually dying from injecting AAS, but it is possible that the coughing fit could trigger a panic attack or exacerbate some other potentially fatal condition (like a heart condition or asthma).
