It is crashed which can be due to a number of reasons. Often it is due to exposure to cold temperatures. It may also be due to the concentration or the solvent used.

The first thing to try would be to heat some water in a pot or in a glass in the microwave. It should be very hot but not boiling. Place the vial in the water with the stopper staying above. Let it sit there and occasionally shake or swirl the vial to help put it back in solution. Once all of the crystals are gone let it sit for about 24 hours and check it. If it has not crystallized again you should be good to go. If it has crystallized, then a solvent such as grapeseed oil will need to be added.

Do not inject it in its current state and do not inject it right after you have heated it. It is possible to crystallize in the muscle even after heating. Those crystals can cause extreme discomfort

Be clear this does NOT mean to put the gear/vial in the microwave...
