It depends on a few variables, but generally constitutional symptoms (fever, lethargy, nausea, chills, hyperhydrosis, loss of appetite) manifest within 48-72 hours. Cellulitis may take a bit longer to develop but can usually be diagnosed within 3-4 days.Originally Posted by oldnsedentary

Oddly, it did not start at the injection site, but about four inches lower, at the bottom of the deltoid, and spread down the side of the arm. It is all the way down to the elbow now, three days later.
No fever, feel ok, still working out, not a lot of pain, but red, hot, swollen.

Sounds like the injection channeled away from the deltoid. Likely causes are that you missed the muscle or it was superficial. More likely a reaction. If the arm stiffens or you lose range of motion, that would suggest cellulitis and require an oral antibiotic to treat (clindamycin).

UGL - high concentration, 300 mg per ml, and this was the first shot from that source. The second shot shows no such symptoms at all.
Day 4 and I think it is subsiding, but that might just be all the ibuprofen I am taking.
The weirdest thing is the migration. Quite literally there is no swelling or redness or soreness at the injection site itself even if I push hard right there. It is all lower. Weird, weird, weird.

If it's red, swollen, and hot then it's an infection. I had one just a few weeks ago and it turned into Accute Cellutis. Go to the Dr. and find out for sure. I'm on TRT so I just told them it was from injecting that. If they ask just tell them you were injecting B12 and you're afraid it's infected. Don't be stupid. If it's infected and you catch it early you only need antibiotics. If left alone it will spread, can enter the bloodstream and kill you.

How long did it take before the first symptoms began to show up? My symptoms began appearing within 8-10 hours, which I am thinking is too early for an infection.
What about you? How long after the injection before symptoms first appeared?

It's tough to say because I was actually away on a snowmobile trip. I did the injection on Thursday morning and noticed it was really sore starting Friday. It was getting worse Sat and Sun and when I got home on Sunday I noticed my arm was all red and hot to the touch. I at first didn't pay much attention to the pain because it was the first time I injected in my delt so I thought it was just PIP. By Monday morning my arm was completely red and burning hot. Never got a fever but once the redness came it spread quickly.
p.s.- The reason I didn't notice the redness prior is because my arm is covered in a tattoo so it was really difficult to see the redness until it spread to a spot where there wasn't a tattoo....
For piece of mind just go get it looked at. If it's nothing then you go home and stop worrying about it. If it's infected then you get antibiotics and it clears and you move on.

I probably would have done this but for the weekend. By Sunday it was subsiding, and today it is almost entirely gone. Whew!

Sounds like an infection to me. I had exactly the same thing the last time on cycle. I went straight to the hospital and got it sorted.

you will know in 2-3days... Go to ER if its hot, red and hurts/itchy (specially itchy) and if you have a fever.... get to er asap
