Okay bud, good question. There are several ways to get an abcess. First off all of what you said, and then some. If you use the same injection site to much it can cause an abcess due to the fact the muscle is over saturated and will start to reject the substance you are trying to put in it. That is why rotating injection spots is crucial. also poor injections techniques can lead to abcesses as well.
Most of the time they do disapate on their one, but in extreme cases some do have to go to a doctor to have it "drained" so to speak.

Did I get an abcess or infection?
Summer of last year, second week, second shot of Deca, into the right ass cheek. Blood squirted out, massaged the area. Turned dark red/purple in that area within 2-3 hours. Then bumps appeared around the area. Bumps didn't go away for 2 months!!!
