Go straight to a physical therapist, and ask for a rotator cuff evaluation, and what he feels should be done. Alot of the tests they can perform can somewhat detect the extent of the injury to the rotator cuff, which is what it is(99% sure). The arm extending behind the back, and up the middle is a HUGE indicator of a rotator cuff tear. Or you can have an MRI done, but that is quite a bit of money without health insurance.
