You could end up in a vein if you don't aspirate. But, sometimes you aspirate, everything is fine, and you still end up with gear in your blood. You will taste it, then you will start coughing. The coughing can be rough, but it is just the oil (from the gear) getting to the lungs and being deposited for clearing out of the system (by coughing).
Aspirating keeps you from mainlining gear, which is bad.
This is rarely a problem. Just be cautious. Eventually you will get the cough, or you will get a bleeder when you pull the needle out. Don't freak if either of those things happen.
