Hi guys. Hoping to get a little help here.
I'm 27. I'm tired all the time, depressed (really flat and grumpy), and libido is missing. This has been going on for about 5 years. My Testosterone is a little low. My Prolactin is a little high. Nothing else seems to be amiss. I have no history of steroid use. Here's some recent bloodwork.
Prolactin 15.6 (ref 4-15.2)
Serum Testosterone 297 (ref 348-1197)
Free Testosterone (direct) 9.0 (ref 9.3-26.5)
FSH 6.1 (1.5-12.4)
LH 4.2 (1.7-8.6)
TSH, Reflex to T4 2.32 (0.34-4.82)
When I did an MRI 4 years ago, they found no issues with the pituitary. I have a varicocele, but a recent sperm test came back looking good.
I don't know the cause of all this. In the past I've had a lot of issues with anxiety, but libido was always fine. It disappeared about 5 years ago after 18 months at a really stressful job and a switch to prozac. Getting away from the job and also prozac for about 6 months didn't change anything. Shortly after I had my first Testosterone test and it was near the bottom of the normal range. If it's plain old depression, therapy and anti-depressants haven't done anything in years. Even my current therapist thinks there is probably something physical going on here with my test results.
I do take Wellbutrin for depression. Not sure if it does anything. I take 1 mg/Ropinirole for Restless Leg. Seems to work. I've been treating sleep apnea for about the last 9 months or so. It's made it so I don't fall asleep everywhere but emotionally and sexually I don't feel any better at all.
Unfortunately all the doctors around here are asshats. I'm stuck with doctors that take the student health plan for now. None of them are willing to help me. Probably doing some treatment on my own soon. Don't feel I have a choice. I'm going to try talking to a doctor who is a friend of a friend soon. Fingers crossed.
Read a lot of studies about clomid being helpful. Am going to start that shortly.
My one other concern is Prolactin. Not sure why that would be high. In the past it was always around 14, so it's been hovering around this area consistently. I was thinking about getting some pramipexole for this. Cabergoline is too expensive for me.
Anybody have experience with prami? How soon would I feel the effect of it if high prolactin is my problem? I also read a study (attached) that cabergoline raised Testosterone as it lowered the Prolactin and also alleviated a lot of sexual symptoms. Would prami have the same effect?
Aside from that it looks like clomid might raise prolactin, so that's my other concern. Most studies don't mention it at all, but I did see one abstract that said it rose by about 22%. This might account for those clomid users who see their T go up but don't feel an increase in libido.
Another concern is that prami looks like it drives TSH down a little bit. Uncool, but I don't know if I have a choice. Also the good news is that prami is sometimes used for RLS anyway, so it wouldn't exactly be out of line for me to try it and I could just stop taking the ropinirole.
Another idea is whether a higher does of ropinirole might help? But from what I've read it's not very reliable for prolactin reduction? Little confused there.
I'm attatching that study about cabergoline. I wanted to put a couple more pdfs up but they don't fit.
So my plan is to use clomid and also a little bit of prami replacing the ropinirole. Thoughts? Suggestions? Experiences? Thanks guys.
