What to tick off in this Bloodwork sheet for free test?

So my doctor checked off the total testosterone. I want my free testosterone to be tested too but i dont find it.

Is there another name or it's included in the total testosterone(which i doubt)?

I don't see it anywhere, and you can't tic what you want, the doc has a copy.

I've done it before and got away with it . Didnt see him make a copy unless he made one after I left..
