How can I be sure if varicoceles are the cause of my low T?

Hi,
I've been having testicle pain since I was 23 years old. I was initially diagnosed with chronic epididymitis. Then I was diagnosed with chronic prostatitis. I've been getting treat for those two conditions for the past 15 years. I would get ibuprofin and ciprofloxacin for the pain. I've seen over 8 different doctors for this. They all followed the initial diagnoses. One doctor, years ago did a testicle sonogram and found a small cyst in my right epididymitis.

Fast forward to to 2014. I find another urologist who decides that he doesn't want to follow everybody else and start over brand new. He sees the small cyst but said it was soft and a non factor. He does an ultrasound again and finds bilateral varicoceles. Grade 1 on the left and grade 2 plus on the right. I also have impaired fertility. I have a good sperm count but the sperm is full of problems (motility problems, bad shape, etc.).

They also ran several testosterone tests and the highest it's ever been is 290. The LH was 4.1 (1.5-9.3) and FSH was 3.9 (1.6-8).

In the end, he diagnosed me with bilateral varicoceles, right orchalgia, male factor infertility and primary hypogonadism originating from varicoceles.

My question is this. Based on all I've read, wouldn't I have secondary hypogonadism since my LH and FSH are low? If I had primary hypogonadism due to varicoceles, wouldn't my LH and FSH be high?

He wants to do surgery but I'm scared shitless of getting cut open. I'm in pain everyday and I've been in pain for 16 years. He said that since I've had testicle pain for so long, that he can't guarantee that he'll restore my testosterone levels, or my fertility, but he can at least get rid of my pain.

I wish I could offer advice but I cant on the cysts and alike. But I had bi-varicolesels (one in each test) and required stents. The surgery was in through the neck whilst awake, prodding around under xray with a dye and job done, bit of ache but not much more but I appriciate you have been having issues for many years. Took about 40 mins for the 8 stents and increased blood flow and testicular size and most importantly for me, increased sperm count and production.

If you are producing good levels of LH and FSH then your pituitary is doing its job surely?

Thanks for sharing your experience. Did your testosterone levels rise? My doctor told me that fixing the varicoceles can cause an increase but it probably won't be enough to get me off testosterone, especially since I've had this problem for so many years and I've been on testosterone for close to 2 years now.
