Omron used to have XXL cuffs for their meters. You have to recalibrate the meter when changing cuffs, but isn't difficult. Basically adjust the valve that controls the air bleed down time.
LifeSource used to make on that came with a XXL cuff already installed, but I really prefer Omron.
If you're used to using the old school manual sphygmomanometers and a stethoscope, you can sometimes find them online. It's a little awkward but it works.
