The general calculation for D is 35 IU per lb of body weight. However, it's perfectly acceptable to start at 5000IU like couch suggested and retest in 90 or 120 days then evaluate from there. Also keep in mind when taking Vitamin D you need to intake both cofactors ; Magnesium (no oxide) and K2 (100mcg). Otherwise you get D leeching calcium from bones without these cofactors.
I split my Vitamin D throughout the day. It's a bit tedious but my philosophy is if I was outside all day I would be getting a stream of Vitamin D like an IV drip. So I try to recreate that. I'm also eating a fat based meal every 2 hours so it's a bit easier for me in that way.
B12 is great but you'd benefit better from a B Complex which covers the whole spectrum. 1000mcg a day is also pretty excessive dose outside a bulking cycle. The type of B complex you want to buy is called methylated (aka activated). In particular, Vitamin B is the key nutrient for your body to regular dopamine and serotonin pathways, so deficient in B6 can lead to things like depression.
I am surprised Magnesium was not checked nor recommended - it's the gold standard to dealing with the majority of your listed symptoms.
In my opinion using L Theanine + Magnesium or even magnesium alone is going to produce way better results than Gabatone. Valerian is a hit or miss placebo, and GABA does not readily pass the blood/brain barrier. A very good product is called Natural Calm (their newest version).
DHEA has a short half life so I would split 26 into 2x 12.5mg. One in morning and one after dinner.
Are your symptoms impacting your sleep quality (aka waking up with brain fog), sleep quantity (hours asleep, # of times awoken), or sleep onset (time to fall asleep) ??
