Have a client who came in a wheel chair. He is unable to stand and claims he is over 600 pounds which I would have estimated at well over 700. I gave him two sessions and introduced a brief introduction on nutrition and exercise. I was never trained on such obese clients. Help me pleAse? I really need anything you know, all comments aside. I would like to help this man. 54 yrs old, at least 50% bf, 600+ lbs. please. Anything you know.

I doubt he is 700 lbs. Not aware of any wheelchairs that work for someone that size. They're generally bedridden. But if that is the case, although I hate this method but in this case it may be justified for him to commit to lipoplasty.

FIRSTLY, you need medical records and you need info from doctor on what he can or can not do.Then you need him to sign a liability waiver...You could get in deep shit here if this person has a heart attack or gets injured somehow etc...Even though it may not be your fault but just because his health condition is so poor...But they can still BLAME YOU and they most likely will...
SECONDLY, you do NOT need to EXERCISE this person as you might think....Work on diet only...At that weight there really is no need to try to exercise..It would be more of a negative then positive...ONLY WORK WITH HIM ON DIET!
At that weight, that is all he truly needs at this moment...When he starts losing weight THEN you can start to incorporate a LITTLE bit of exercise...
As weight loss progresses increase exercise regimen...But let me tell you, this person don't need no exercise until they lose over 200 lbs..
Which EASILY be done with just diet alone at that weight...

And I would honestly whole hearted take my advice...
Before you do anything else with this person you need what I said..Medical records and docs orders written on paper along with liability waiver he needs to sign that with release you of any liability..
Also, NO EXERCISE! PEROID! Just work with him on diet....
And to tell you the truth..I bet tis person is just WAITING to blame someone like you yo get money from a lawsuit...WHY?
Because someone at that weight OBVIOUSLY has MAJOR health issues and is OBVIOUSLY working with a doc and nutritionist....
So why would they try to go to a personal trainer when they are working with Certified doctors?
Furthermore, the BEST thing you can do with this client is MOTIVATE them (Perhaps, that is REALLY what they are looking for)..MAKE them believe they can lose weight.Encourage them...Doctors see MANY MANY people everyday and are usually strapped with time and DO NOT CONNECT WITH PATIENTS ON A PERSONAL LEVEL...
THat is where you can come in...Bring in pictures of people who were overweight and lost A TON of weight...Even if it is not real...Encourage them and support them....
Let them no that ope is NOT lost..Which it isn't...ANYONE can lose weight even the obese...Last edited by calstate23; 06-18-2013 at 02:17 AM.

sorry.
op, may i ask what profession are you in? what sort of client is he?
i would think for someone who is so morbidly obese, he may need a full medical evaluation, and maybe medical and surgical treatment.

Again, TDEE is INSIGNIFICANT at his point....
Firstly and foremost this person needs motivation and encouragement BEFORE anything else....If you treat him like the doctors and just spit out numbers it will go in one ear and out the other...They're looking for a connection...
Most of these people don't get out or even have many friends etc...Be friends with them and ENCOURAGE them..They need a goal and he probably isn't going to set one himself....
Change this person mind set and bring the info when they need it...Nothing better then helping over weight person achieve their goals and change their mind set....

He does when the time is right..And it's not now...
I always look towards positive vs negative...And the fact is (If this shit is even a real topic) that exercising this person is more of a negative......
For legal reasons and also for the person health...I mean, if this guy does even bicep curls he probably never even touched a weight and going from not touching a weight to lifting weights equals NO BUENO!

The ONLY acceptable exercise regimen I would perform on this fella is rubber band resistance...If he wants to use rubber band resistance on a LIGHT weight I agree with that..But that's it..

I appreciate OP making an attempt to help this guy. But I think he is wasting his time. The man needs surgery, nothing else with help him. He will not become the 'Miracle Man' on the news that lost 500 lbs.

I hate it, too as I stated in my first post. But this is mission impossible. I don't care how motivated you are. I'm just being realistic. As for health, his health is in tremendous danger as it is. I'm all for getting the job done right, but man.... forget all that noise. Google 700 pound man, I don't think a lot of people understand the severity here. I still think the guy is not 700 lbs. He wouldn't make it to the gym.

Lol...But the fact is whether he gets lap band to reduce caloric intake or does it himself it's still the same thing...Certified obese people just ave no hope, plus bad genetics, PLUS have TRUE hunger issue that most haven't experienced...
But it still comes down to one thing and that is AT LEAST TRYING to progress..
It's a slow process with these clients...If you can just get them to not drink a 12oz soda pop a day then they've already progressed...Just get them to do little things everyday and every week and they will progress...I've seen OBESE clients like this...Have I got through to ALL of them? No, but a lot I have....

What is 'Certified Obese People'? You mean medical/genetic issues?

Lol..I have seen this guy before and he was lazy shit...(His girlfriend was SEMI DECENT for him)....
The biggest factor is lost hope WITHOUT a doubt...They think it's not possible, people look at them like they're pieces of shit..They give up...They say, shit, "Im never getting a girlfriend/boyfriend etc"....
It's possible 100%, I've seen it PLENTY OF TIMES...

How do I block some people's posts? I find their stupidity disgusting!
On topic - I would say at this point all you can try and do is educate them on correct nutrition. Possibly suggest a simple diet around 2500 cal - but unless you can find a way to get a BF% and do it accurate. Just guesstimate, educate and motivate.
He has to help himself. Does he understand the very likely high risk he's put himself at?

The BAD genetics and hunger problems is true....
I DIDN'T say they have NO HOPE (Sorry if it came off that way).......What I was TRYING to say is that THEY have no hope.....
They think it's not possible to lose weight when in fact they CAN lose weight..And A LOT of it......

I agree. Such cases need to be treated with some urgency. Almost definitely they will have a number of medical problems. Diabetes, blood pressure, cholesterol, ischemic heart, hypothyroid, fatty liver... i would take just enough time to medically stabilise all these problem and put him under the knife. Gastric banding or gastric bypass can only get him from 60%+bf to maybe 30%. We can then take our time to decide what is the best way to get him from 30% to 18%.

Gotchya. When you say you've seen folks lose weight 'Plenty of Times', are you referring to overweight people in general, or 600 lb+ men/women? I ask because I personally do not recall the last time I saw a 700 lb person. Most certainly not at the gym and I spend plenty of time at tons of different gyms.

Yeah, just UNDER maintenance....Not a MASSIVE AMOUNT UNDER MAINTENANCE....
Dropping his caloric intake that low would NOT help him....
Firstly, yes, this client doesn't care about building lean muscle tissue while losing fat at the same time...Lol...This client just prays that MAYBE ONE DAY they will get under 400 lbs..
What happen to a normal person when they drop caloric maintenance DRAMATICALLY is they lo9se water and muscle....
And, HYPOTHETICALLY, you would think that is great for someone that doesn't care about building muscle tissue and getting lean...RIGHT? They just want to lose weight..
But the thing is IF they ACTUALLY did that...They better STAY On that diet to a T or else they gonna BLOW UP like the Michelin Man once they get off....Which they will...

There is no way on Gods green earth that this guy is anywhere near 50% BF. Not a chance.

It would not take years at ALL...In fact, it will drop DRAMATICALLY if you can motivate client enough to do it...
They will drop HARD AND FAST..Then it will crash..That's when the ol' motivation come back in.....
Have I seen 700 lbs client? No, I haven't BUT I have seen 500 + MANY MANY TIME and they are basically in SAME situation...
I don't care if someone is 900 lbs they can be 98 lbs....Just google concentration camps..Last edited by calstate23; 06-18-2013 at 04:46 AM.
