Ok, so this is pre-treatment. You need to give blood asap. If a donation center won't take you have your doc write a script for a therapeutic draw.
Your E2 level is way low! How is your libido? Any joint pain? As your T rises E will follow it so that will eventually help there. Nice to see the proper assay run for it as well! PSA is great! IGF-1 I'd like to see higher but give it some time to see what happens with your therapy.

This is bloods after 4 months of 65 mg test cream and 100iu hcg per day. Did you mean pre clomid treatment which I am switching to now. I wanted to do it first but she did not, she talked me into cream instead. After 4 months on this board I talked her into trying clomid
Pre cream treatment 4 months ago all I had is the numbers below. She started me on the cream without full tests, Ive since learned better.
Testosterone Serum 323L ng/dl
Testosterone Free 6.69 ng/dl
% Free Testosterone 2.07%
That only brought me to the 409 so I decided that with my super low estradiol level to try clomid. Which like you said if it works will raise my estradiol on its own. Yes libido was inconsistent, ejaculating was hit and miss and joints have hurt like a mother.
I asked her if i needed to give blood and she said no. Is it absolutely necessary?
Also if I respond poorly to clomid or it does not increase test she has already given me a script for either.
The first option is to increase my topical cream of 65 mg test/progesterone to a gel of 200 mg test/.5 mg estradiol/8 mg progesterone.
The 2nd is a sublingual pill/pellet of 200 mg test/.5 estradiol/8 mg progesterone.
I hope I don't need to but if forced I will probably try the gel, the sublingal sound like too much of a rush of test then followed by withdrawal.
Thoughts?

BTW my fault I have other threads with questions but never had full lab numbers in them and wanted to see if anything on them jumped out to people like hematocrit and numbers.

Your first post was very easy to misread. Research polycythemia regarding whether you should give blood or not. Out of the options your being given I'd go with the gel. I assume it's a compounded product. Don't even think about a sublingual pill. That's ridiculous.

Thanks I really appreciate it and yes it was confusing. Since the cream and hcg raised me so little i really hope 25 mg clomid eod raise me to 700 or so. 700 would be unreal considering i felt tons better on the cream minus the joint pain. If a 100 raise did that another 300 would feel like Im on cycle.

Have you talked to your doc about self-injecting as an option as well?

Yeah I think if clomid doesn't work I will end up doing the sub q that Dr Crisler recommends. She will do whatever. She usually either does the compounded creams or self inject. She hates the clomid idea but i talked her into it. If it works its the easiest and cheapest to do.
