My blood test two months ago showed a T level of 638. I know that optimum would be that of an early twenty year old, 1100 ish. I'm 39. My question is: what is the natural standard for a man my age?

actually the average level of a 20 year old nowadays is around 700, someone around your age would probably be around 5-600 ;)
You must go by how you feel. It varies for each individual. Some feel good in the 600's. Some feel good a 1000+.
Put the emphasis on free T, not total T. You can have all the total T in the world but if it's all bound it's not doing you any good.
