Has anyone started TRT in the 400's ngdl range? (13nmol-18nmol)
Has anyone started or does anyone know someone who started TRT in the 400's ngdl range? (13nmol-18nmol)
Im 20 years old and i have had 2 tests, 420ngdl and 450ngdl. Free T middle of the range on both. Good E2 aswell. Cortisol High but in range.
The thing that confuses me is i have pretty severe symptoms and they have slowly got worse over 2 years;
-Brain fog/slowed thinking.
-Erectile dysfunction with completely no morning wood.
-The no morning or night wood makes me urinate like mad at night.
-Anti social even though i know deep down i love social interaction.
-Lack of drive and motivation for ANYTHING.
-"Andro-Gut"
Any input would be welcomed. Really don't know where to go anymore, such a shame you can't just 'try' TRT.

If you're numbers are in the middle of the range on both, what has your doctor said? At your age it is very possible these may be the symptoms of something else.
I would definitely see more than one doc before starting TRT at your age. I also would highly recommend not self treating. This may be the symptoms of something that easily treatable...definitely see more than one professional.

I had total T levels at 440 and free T at 10 out of a range of 9-25 i think it was at the age of 20. depressed, no energy, gyno, and feeling weaker and weaker every week in the gym (keep in mind I still trained my ass off, have for 7 years now). Always eating "clean" and avoiding things I realized werent good for MY body, yet still had all the symptoms.
First dr told me I was just depressed and needed to see a Psychiatrist (LMAO laughed and basically walked out).
Second dr was my primary and he said while I wasnt clinically "low", he understood where I was coming from and knew why I wanted therapy, but he couldnt do anything for me.
Third dr was an HRT specialist and she helped me out and Ive felt the best I have in my entire life.
I know how u feel buddy, keep trying new drs, try googling for a hormone replacement dr in your area, even if u have to make a little drive, do it. its worth it. A 20 year old shouldnt have ED, and i know the feeling, its the worst feeling ever.

If your free T is midrange then you don't have low T. I would look at other things. How's your thyroid numbers? Hypothyroidism has basically the same symptoms as low T. High cortisol will also contribute to these symptoms and will push your T down.

Hi guys, appreciate the mature replies!
Score how do you feel now after starting TRT?
Also for those who asked my thyroid labs are;
Serum T4 – 14.4pmol/L - (9,10,12 - 18,23) < wikipedia reference says it could be any of the 3 lows or either of the 2 highs.
Serum TSH – 0.58 mU/L (0.30-5.00)
My morning cortisol was; 532nmol (140-700)
Just sent off my saliva cortisol + DHEA samples to genova (private online testing company) to see if my saliva cortisol reading is low or high.
Going to book a doctors appointment as soon as possible and ask for a full thyroid test;
Free T3
Free T4
Reverse T3
Reverse T4
TSH
Is there anything else you guys could think of? I have other labs here but nothing raises the eyebrows;
SHGB – 25.3nmol/L (15– 48)
Free Testosterone – 62.1 (34 – 106)
Total Testosterone – 15.7nmol
Serum Oestradiol – 73pmol (50-200)
Serum LH – 4.1 IU/L (2-9)

Hey man thanks for your input.
If i was to go onto TRT i think i would make sure i was hovering around the 1000ngdl mark. I mean 20 year olds should have around 600-1200 ngdl anyway from what i read on sites like this.
How much mentally do you think 400ngdl to say 1000ngdl with good E2 would differ? My get up and go is right down the drain at the moment.

Hey man thanks for the reply.
My free T was 76 on a scale of 36-106
Do you know if they actually do a proper test for this or do they just work it out from SHGB and albumin levels?

Have to add the second test came in it 62 on the same 36-106 scale if that makes any difference

Nobody has mentioned possible high Estrogen. Have you had blood work on E2 sensitive essay??
May be a possibility

if you do this correctly, then you do NOT know where you will be hovering at. Again, with TRT, less is more. This is a true statement. Because TRT is a lifetime decision. And the higher your levels, the higher your sides. Instead, think of it as either a balancing act, or one big compromise.
of course, you want the highest possible levels. But your long term health demands the lowest possible sides. It may take a couple of years to figure out and dial in your ideal dose. It may be that at 1,000, your RBC and other values are doing fine. And in that case, you are good to go. However, it may also mean that at 1000, your RBC and BP, amongst other things, is "through the roof", and you need to reduce.
You will not know what your ideal levels are until you go through this process.
My values are in the mid 700's, and I'm still struggling with elevated RBC levels.
So please do not try to assume what your ideal levels are. Your health depends on it.

Hi guys thanks alot for your replies as always, appreciate it!!
Just got the results back from my saliva cortisol test;
Morning level: 19.7 (12-12)
4 hours later: 8 (5-9)
4 hours later: 5.2 (3-7)
prior to sleep: 4.2 (1-3) * Flagged as high
DHEA : Cortisol ratio 1.89 (2-6) * Flagged as low
DHEA mean: 0.70 (1.47)
Going to get full thyroid blood work as soon as i can.
Lack of sleep and night sweats is getting to me at the moment. Surely a level of 450ngdl couldn't make someone feel this bad?!

join the club. we all have our days......
there could be other/multiple reasons for your symptoms. it almost sounds like some of this could be due to sleep apnea. this condition is more common than you think. There is really no way to know if you have sleep apnea without some type of monitoring while you sleep. There is a device that clamps to your finger, straps to your wrist, that monitors, amongst other things, oxygen saturation levels while you sleep.
Some clues you may have sleep apnea:
Do you snore?
do you get up multiple times in the night?
do you toss and turn when you do sleep?
do you wake up feeling tired and generally unrested?
You may not have this condition. But then again, you might. And if you do have this condition, it needs to be treated.
Do your internet research first. if this sounds like maybe it might apply to you, then it's time to have a chat with your doc.
