    Opinion on my Blood work please

    here are the results of my blood work performed recently. The normal reference range mentioned in results are high and does not give an accurate idea. looking for opinion on the results and suggestions for improvement.

    male 37 - moderate gym workout 5 days a week.


    1. TSH 2.63 uIu/ml reference 0.35 - 4.94

    2. LH 2.99 mIU/ml reference 1.14 - 8.75

    3. FSH 2.22 mIU/ml reference 0.95 - 11.95

    4. Testosterone 384.80 ng/dl reference 300 - 950

    5. Free Testos 6.72 ng/dl reference 9 - 30

    6. HBA1C 5.2 %

    7. Glucose (Random) 82 mg/dl reference 80 - 200

    TSH is a bit elevated actually which could impact T levels. The range given is an older one. More modern is .3 - 3.0 and pretty much anything over 2 should be looked at and monitored. Keep an eye on your FT3 and FT4.
    LH is in range but lower which ultimately means your T will be lower.
    Free T is abysmal. Free T is what works for us and yours is below range. Would be nice to know your shbg level here.
    Cortisol and prolactin would have been good to see.

    If thyroids not impacting levels and cortisol and prolactin are not an issue then it may simply be time for hormone replacement.

    I concur with Kel, most of the tests look fine but Free T is very low, which indicates high SHBG. I'd have that test specifically run to validate and also to repeat the Total T at the same time. It's on the low side, but not out of range. Could be just a normal fluctuation in the test results. Normally, Total T is at the higher end of the range if SHBG is high, as it protects T from liver metabolism.

    After you have those tests run, come back with the new results and we can give more definitive advice. In the absence of liver damage, high SHBG is a genetic thing. It affects about 10% of men and the symptoms generally manifest themselves as ED and low energy in your late 30's and get progressively worse as you age. There's not a lot you can do to treat high SHBG within the confines of mainstream medicine. I've gone off the reservation for a less conventional approach that is VERY effective involving very, very low dose oral anabolic steroids in combination with standard TRT. You can find discussions of it in several of my posts in the last year (search on Winstrol and/or Anavar ). However, get the tests done before you take any action.

    as recommended, here are my latest blood work results.

    Thyroid Function Tests

    T3 1.1 ng/mL Range 0.6 - 1.6

    T4 7.6 μg/dL Range 4.9 - 11.7

    TSH 2.30 μIU/mL Range 0.4 - 4.5

    Prolactin 6.2 ng/mL Range 3.5 - 19.4

    SHBG 34 nmol/L Range 14 - 49

    Testosterone 336 ng/dL Range 249 - 836

    Free Testosterone 5.94 ng/dL Range 9-30 (1st week of jan 2018 it was 6.72 ng/dl )

    i have had a an excellent libido and sex drive throughout my life but i have been facing very low libido and ED since i started using 2.5 IU Pharma Grade HGH after christmas. I do work out 5 days a week and eat relatively healthy. last year when i was using HGH for a few months i did face some low libido and ED issues too but things were ok again when i stopped using HGH. i am 100% confident that my source and HGH in 200% legit as i get it from the main pharma distributor. almost all the reviews and benefits of HGH include improved sex drive but mine is the opposite. please guide!!!

    Nothing is jumping out as a cause imho. Even if it were head or testicular trauma it could be detected on your BW in LH/FSH values and T levels. It's great to see you put in the effort to find a cause but unfortunately nothing is rearing it's ugly head. It may just be andropause and time to initiate a solid TRT protocol. At least you can enter into it with a clear conscience.

    When it comes to GH consider splitting your dose in half. Once in the morning and one in the afternoon. See if this helps. If not better then discontinue it again and see.

    Pretty much the same input as Kel. Your LH/FSH from your previous test does not indicate a problem with the hypothalamus/pituitary (i.e., secondary hypogonadism). Since your Total T is in range, it does not indicate primary hypogonadism either. Your SHBG is in range too, but it's the balance of Total T and SHBG that is your main problem. Your Total T is being sucked up by the SHBG and your Free T is low. It's the Free T that is important. I see 2 courses of action:

    1) Increase your Total T with TRT to get your Total T in a range that enough spills over to keep your Free T within range. I'm guessing that with your fairly moderate level of SHBG, that should be easy to do with typical TRT protocols. I recommend starting at 40 mg of T-cyp Every 3 days and then recheck labs after 6 weeks and see if you need to bump it up (or not). I DO NOT recommend any sort of estrogen inhibitors unless your 6 week labs indicate that it is needed. Make DARN sure it's the correct lab too. My guess is that your E is on the low side now (because it's made from T) and that is probably contributing to your ED issues. Guys need E too.

    2) If standard TRT doses are not sufficient to raise your Free T sufficiently, you can suppress SHBG production with either Anavar or Winstrol , but this requires black market sourcing and you have to do TRT anyway since these synthetic steroids are suppressive to endogenous T production. I DO NOT recommend you try this type of therapy unless you first exhaust option #1. It's really only necessary for guys (like me) that have way excessively high SHBG.

    Regarding GH and ED. I think Kel gave some good advice, try splitting the dose and/or cutting back on the dose. I've not heard of guys having ED issues with it, but if you think it's causing your problem, then you need to alter your protocol. Some guys (like me) do not react well to GH supplementation. For me, even small doses (1-2 IU/day) causes significant water retention and that affects my overall mood and energy level negatively. I had not noticed ED issues, but if you are retaining water and if it's raising your blood pressure, I could see that happening.
