My very simple layman's distinction between primary and secondary:
Primary - some part or parts ain't working and it can't be fixed
Secondary - things are working, just not as well as they could
NOTE: you really ought to google these terms and get much better definitions.
At any rate, in the first case, you don't have much choice except to bring in external test. In the second, you have options.
That said, if your goal is to maximize your hormonal profile, you probably ought to marry the needle (ie proceed as if you are primary). That route opens up all sorts of options for other complimentary suppressive drugs.
Treatments like SERMs (clomid for example) are intended to enhance your natural production. And they can do pretty good jobs of that. But they are only useful as main HRT drugs in cases where secondary hypogonadism is the root.
All that said, SERMS < AAS (including good old test). Personally, I will likely be on the needle in a year or less. There are too many variables I want to control that SERM therapy just doesn't offer to me.
