If he says "Yes", next question would be:
Can you specify which particular medications or drugs, illegal or otherwise, you'd have to squeal about?
If he says "No" to this, then I'd just leave without paying (although that can be a little tricky in an embassy with gates and armed guards :P)
If he says No to the first question, or if he doesn't mention steroids in the second question, I'd start telling him about the juice.
I'm in a part of the world where the only good doctors are inside foreign embassies, so I'd be wise to figure out beforehand if they're gonna squeal on me. The local doctors here have trouble with First Aid, let alone doing blood tests.
