I know that I have to move my treatment, just want to make sure that I am going with someone progressive that knows their stuff. I will stick with Crisler. The scary part for me is the cost for the other Dr's. Right now this is all covered by my insurance. Looking at Crisler's and Maximus's prices spooks me.
John's office will provide all the insurance codes you need to submit claims to your insuracne company so you should still be covered.

What type of health insurace coverage do you have? If not an HMO/PPO you should be fine.
