modern medicine makes it nearly impossible to tell the difference. once post op, even more difficult to tell. those birth control pills are not for stopping pregnancy, so just because you see she has them doesn't mean she is genetically female.

if a male starts with the hormones early enough, once post op, even an Xray of the pelvic area makes it difficult to tell in some cases.
