GD's BW - Have at it boys!

Ok, so I got a very comprehensive blood work done and labs today. I am not going to print out everything here like all CBC and General Chem assays as they were all in the mid to optimal range and really don't impact out TRT protocols. I am sharing what I believe is the most relevant assays for your review and input. I have some general observations below as well.

    CBC
    Hematocrit 46.2 Range: % 38.5 - 54.0

    General Chem
    Albumin 4.8 Range: g/dl 3.2 - 4.8

    Coronary Risk
    Triglycerides 110 Range: mg/dl <150
    CHOL, Total 191 Range: mg/dl <200
    HDL 70 Range: mg/dl >40
    LDL 88 Range: mg/dl <100
    CHOL/HDL Ratio 2.7 Range: <5.0
    Homocysteine 8.5 Range: umol/L 4.5 - 15

    Thyroid
    TSH 1.15 Range: uIU/ml 0.27 - 4.2
    T4, Free 1.32 Range: ng/dl 0.9 - 1.7
    T3, Free 3.6 Range: pg/ml 1.8 - 4.6

    Tumor Marker
    PSA, Total 0.78 Range: ng/dl 0.0 - 4.0

    Endocrine Evaluation
    Progesterone 0.92 Range: ng/ml 0.2 - 1.4
    Estradiol 21.3 Range: pg/ml 7.6 - 42.6 (Sensitive)
    DHEA-S 228.5 Range: ug/dl 44.3 - 331.0
    DHT 47.72 Range: ng/dl 11.2 - 95.5
    Test, Total 1133 Range: ng/dl 280 - 1100
    Test, Free 30.50 Range: ng/dl 1.6 - 22
    Test, % Free 26.2 Range: ng/dL = 2.53 %
    Test, Bioavailble 681 Range: ng/dL = 66 %
    SHBG 26 Range: nmol/L 10 - 80
    IGF-1 195 Range: ng/ml 87 - 238
    Cortisol 16.1 Range: ug/dl 4.3 - 22.4

My Observations: Pretty much what I expected. I don't pay much attention to Total Test at this point. What is important are the two test assays highlighted in green. Test % Free - 2.53 is spot on as is Bioavailable. E2 is damn near perfect as well. I would like to see DHT near the higher end of the reference range and may trial some DHT cream to see what it does. It should be higher in my opinion with where the Test panels are falling. This is the first time we tested for it so I have no bench mark but I have suspect it was low.

Thyroid looks healthy and converting T4 to T3 nicely.

I've been dialed in for some time now but it all looks pretty solid to me and is pretty consistent with past labs.

Let me know your thoughts Gents and if you want to see any other assay; but as i said, they are all within the normal reference range.

    Thanks guys!

    Thanks man.

I've had lower Cortisol levels in the past; it fluctuates with stress.

I happened to be getting a cold the day we drew blood so it could have been that.

Thanks b, means much coming from you!

    Protocol:

    120 mg T Cyp (60 mg twice a week)
    250 iu hCG EOD
1 mg AI (split .5 mg 12 hours after each Test injection)
    50 mg DHEA Cream Daily
    50 mg Pregnenolone Cream Daily

Total and Free are a tad high but nothing that me and my Doc worry about. We look at % Free and Bioavailable as our indicators and they both are dialed in bass.

    I hate you!

But seriously, I know you've devoted alot of thought into dialing-in your TRT.

I have nearly the identical protocol as yours (- the creams), so it's definitely a proven formula.

    Congrats.

I did notice changes when I added DHEA and Preg in the cream. Felt a little euphoria (some may say anxiety) but once my body got used to it all was fine.

I should add that I also take 3 mg of Selegiline with these two as well.

Yes, Test and E2 couldn't be better.

I shoot 60 mg Monday morning and 60 mg Thursday night.

Test was done on Wednesday morning 8 am. With my Protocol my levels should be pretty consistent all the time so there's no day where I am "down".

    Interesting...

Do you favor a specific protocol? Prior to this last draw, what was your history the previous couple of weeks?

Looks great gedv. A great example that shows those that are proactive and get invoilved and do research and self advocate for proper treatment can seriously help themselves. Thanks for sharing your knowledge and experience here with the forum gdev!

HDL is off the charts. Good sign for longevity. Anything besides cardio and genetics bumping that up?

It's been at this level for years!

I take a pretty robust supplement protocol so it may be caused Fish Oils (omega 3 fatty acids) and I do like a few evening cocktails as well (yes, alcohol can/will increase HDL!). I also add soluble fiber to my coffee every morning.

More likely genetics than anything else Chem.

Here's an interesting quote re DHT:

"It is important to understand that even though testosterone is the active androgen in muscle, and DHT exhibits relatively little direct anabolic effects on muscle in men, DHT is still very important for the full performance enhan***ent effects from testosterone. What I specifically mean here are the effects of DHT on the central nervous system that lead to increased neurological efficiency (strength), and increased resistance to psychological and physical stress—not to mention optimal sexual function and libido."

T is in the muscle, DHT is everywhere else.

Great quote kel; where'd you find this? Is there more?

J - Like all hormones optimal levels is key for all things we know here and DHT is one of them. Like DocM said, it is in some ways the "forgotten hormone" yet it is all things male!

Too low levels = Female traits. Too high level = Hair loss and other things.

Plays a huge role in libido!

DHT can be added to a HRT protocol through the use of a transdermal cream; can even be bought online over seas.

I am mid range on my DHT assay and I am investigating where the optimal range is. I haven't had a conversation with my Doc yet, but even DocM didn't seem to know.

I am really looking forward to his response on this as well.

I am tempted to trial some DHT cream for a month to get to the 80%+ range rather then the 50% I am sitting now. I've been really curious about this hormone for some time and anxious to see what a trial would do for me.

Did the gel route for a few months and my DHT shot up to 4x normal (due to the 5-AR enzymes in the skin). Can't say it did anything for me other than greasy skin and some mild acne.

My theory is you want to either: (1) keep it well within range, or (2) ignore it since it's part of the HPTA feedback loop and screwing with it while on TRT could be detrimental.

I could see its possible use in lieu of TRT but for us, I'm not sure it has any special function that we would want to manipulate - at least nothing that test doesn't provide.
