i think cycling intenity is important to keeping motivated to go back to the gym, but every set to failure seems to be what gets me growing, so i mostly stick to that.
After my warm-up sets .. I go to failure on everything, it works well for me. And it helps me in keeping mental intensity, which is a key in training (atleast for me it is).

Same here. I have a set number of reps for each warm-up set, but as soon as the real workout comes, the intensity shoots up and I go untill I can't go no more.

2 for each muscle group right now. I'm too wiped to do any more.

1 warm up set for me. Then I go 3-4 sets (depending on the exercise) to failure...
oh ok, well because right now my trainer is trying to lean me down and then get me bulked and that's why he doesn't want me to fail every set, maybe the last two. But then my brother whose been in the gym for a while and knows what he's doing also, told me that I should fail every set or at least 4 out of five like what MBaraso said.
I'm the same muscle failure is a must.I do 3 to 4 sets also.
me personally i like pyrimiding the weight up
i hack out 12,10,8, and then two more sets to failure
this is what works for me
IMO i perfer bulkin up completely then cutting off the extra weight
