This depends on your goals and your current stats.

This depends on what compounds your running. Short esters are fantastic for blast cycles. 10-12 are common for long esters. Whatever your goals are will determine which method is better for you.
3- Is it okay to inject Test E 2x weekly in the same muscle? IE glute.
Rotating spot injections would be wise. One injection in the left glute, one injection in the right. Common protocol there.
