It's a local place you go to, they typically accept insurance, just like any other place. Whether they pay or not, that's carrier dependant.

So insurance covers you when you want to get a blood test willy nilly without a doctor's request?

Mine doesn't. But we're talking $100 out of a cycle that costs over $500 or more.
Edit: even if I went through ins I'd have to pay a copay and a lab fee, which would about to around $50 if I remember correctly. Not with the hassle.

Well all I remember last time I had bloods done it was just regular liver kidney cholesterol stuff and I didn't pay a thing not even a copay at the lab, just the copay when visiting the doc that requested the labs.
