need advice on current TRT

I am 38 years old. I was diagnosed by my primary doctor with low test. The reading was 245. This was in September. I started with Androgel one pack a day 5mg. I felt really good after about 3 weeks. Then the doctor called and said my first blood test had my test getting too high. I think it was around 400. So he cut me to a dosing schedule of Sun, Mon, Wed, Fri. My level then stayed around 375. The doctor then said we could go up a little and told me to add every other Saturday. Well after about 5 weeks my level had jumped over 1000 and he told me to cut out the saturday dose. My problem is that after we cut the dose after the first 3 weeks I was on therapy my testicles have shrunk and my sex drive has went down. I have been on therapy since September. This is my primary doctor and I asked him about Nolvadex and he did not seem to know what this was. I was considering going to Oasis to see if maybe they would be more informed on treatment options. My insurance has been paying for treatment. I don't know if they will at Oasis. What should I do?

Before they perscribe anything there has to be a reson. Get your estrogen
level checked. If your total estrogen is over high end 130+ Your test
in aromatizing into estrogen. It means you need an aromatase inhibitor like the one you mentioned or arimidex-anastrozole by AstraZeneca. It will increase your test and reduce your estro. It is expensive unless you got insurance for drugs.

i don't know anything about oasis however swale has his androgel patients using 250iu of hcg every third day to address testicular atrophy.
androgel also comes in a pump dispenser and as far as i am aware 2.5mg sachets also.2 pumps equals 2.5mg dose.you could ask to go with 2.5mg/day rather than the every other day protocol.
androgel is designed to be absorbed over a 24 hour period.i feel missing a day is not the best option,instead a reduced dose every day would give you a similar weekly dose without taking 'off' days.

also UFA, coach669 mentions nolvadex which is a SERM and not an AI.

Coach still trying to figure out how your level's (test) got to 1000 with gel.
Did you take it before the blood test. For me the gel works great for a couple of hours. We all have different asorbtion rates. When I was on Gel I would
wait 48 hours after last dose for blood work. The place you mentioned you should definately at least call. They deal with these issues continually. I think they even have a toll free number.
