All you have to do is send them two mL (two cc) and a $100 money order per test. Write a description of the product on each sample (ex: testosterone cypionate). They will email you back a pdf file of the lab results and then mail you a hard copy that's signed on letterhead.
Do not send your whole vial--you won't get it back. Only send a sample.
