No, FDR was elected 4 times. There was no rule about term limits before he did that. It was just a precedent that Washington set and nobody successfully broke until then.
Thomas Dewey (the guy that the newspapers declared beat Truman in the 1948 Presidential election, but really lost when the votes were actually counted) as part of his campaign said he would push a constitutional amendment through Congress. He didn't get elected, but the lobbyists still got it pushed through Congress and 41 states individually ratified it (only needed 36). And so it's the 22nd Amendment to the constitution. So there was no "executive order" that changed the rule to two terms.
