From what you are telling us it most likely means 250mg per milliliter and there are 10ml.

Sounds like a New UGL. Either lab max it or ion it and see if it's legit. UGL's come and go, some faster then others so you don't always get much info off the net. Try taking a pic of the vial and posting it in the brand discussion forum.

Is it a 10ml vial or a 25ml vial? Looks like a low budget UGL to me.

according to the label its a 25 ml vial. I'd estimate that ATLEAST that is correct.

Looks fake but only way to find out is cycle it or get it tested

Bad label for sure. Bad gear? Don't know, you need to either try it and see if it works for you or dump it and get a new vial.
