I'd say alot of it is people knowing how to stimulate muscle growth. People who train really hard and know how to keep up the intensity, they know a crap load about dieting, and have some good genetics. We see alot of people in the gym working out, but their form can sometimes make it so they're actually working a different muscle that they didn't even want to, without them knowing it. We see alot of people in the gym who lift regularly, but take WAYY to much time between sets, and don't nearly go to failure, and they wonder why they don't grow.

Alot of the times, it's not the peoples fault who aren't growing. It's magazines and ads etc. telling them that if they do a weird type of pushup 20 times for 10 minutes that they can look like the people on the front cover of the magazine. Of corse theres the cases where people aren't eating nearly enough for muscle growth which is a large population.

So i'd say a mixture of hard intensity training, knowing your body, good genetics, dedicated dieting and being an informed consumer about the fitness industry and not falling for the new "fad" all time time AKA just simply doing the things that you know work.
