i'd attribute the #1 reason to bad luck.

mostly everyone covers their tracks in some way, however bad things just happen to good people all the time. there isn't some "top" way that it happens...you can do alot of shady **** and never get caught, or get unlucky once and go down.

its a gamble, but for most it pays off.
