I think it depends on what you are trying to do. When I was younger and did a lot of powerlifting-type training, I wore a belt on every set except warm up. I never went over 6 reps and rarely over 5. The goal was to move as much weight as explosively as possible.
Now, I usually don't use a belt. I feel like squatting (and deadlifting) without a belt helps to strengthen the stabilizers involved in the lifts. These days, I concentrate on the quality of the contraction rather than the total weight moved. I only use a belt when I am going as heavy as possible in the 3-5 rep range with the goal of concentrating solely on the primary movers. I don't do this very often because of the recovery time and risk of injury involved.
