Technically there are 46 states.
Virginia, Kentucky Pennsylvania and Massachusetts are all officially Commonwealths.
This grants them no special constitutional powers. They simply choos this word to describe them selfs at the end of the War of Independance. It made clear they were no longer 'royal colonies' answering to the King, but states governed by the 'common consent of the people'.

Virginia (named after the 'Virgin' Queen Elizibeth I), as DSM has pointed out, one of the original 13 States or Colonies (hence the thirteen red stripes on American Flag) and was the first state to declare its self a colonie in 1776.
Pennsylvania and Massachusetts followed suit shortly afterwards, and Kentucky, which was originally a county of Virginia, became a Commonwealth in 1792.
