i would tell him, but he'll figure it out if you don't i would imagine

will it go on your record? that a tricky questions, legally there is doctor patient privilege which means that he can't divulge information about you to a 3rd party source, but there is a loophole in that if he thinks you will continue to do something to hurt yourself

for example, a vicoden addict goes to a doctor and asks for some pills because she is in pain, she asks for vicoden and after talking with the doc divulges that she is an addict and wants the pills for recreational use, the doctor can divulge this info to the police and have her arrested and put into a rehab center under the guise of "the greater good"

though if you say you made a mistake, you don't want to take AS anymore(and you shouldn't) and accept responsibility then i'm sure the doc will help you
