I used to have the same problem. My question is do you play a sport? The reason that I ask is because I used to get the bench shoulder because I would play hockey on Friday night and go to do chest on saturday. I think this was because in hockey a lot of your movements with the stick use your shoulders so I would get sore when I would go to bench. I still get it sometimes but mostly I got used to it. But if its really a problem, then I wouldnt do it anymore. There are other alternatives.

yeah i would give db a try for awhile and do not pull elbows back or as you say flare out, chest out, shoulders down and keep elbows angled, when you have flared out or at 90 degree there is much more stress on shoulders and you are actually working chest less. goodluck.

I fixed my form and lightened my workout and last week. I haven't had any more problems.
