Considering injecting testosterone shutsdown systems (HPTA), I would think that the preferred thing to kick start is the pituitary's LH production or the hypothalamus' GnRH production which will start LH production.
Either way, hormones are not a pediatrician's specialty. Check with an endo, one who has worked with puberty issues. Go from there.

I wasn't sure what kind of specialist was meant, and such I didn't put a lot of stock in the pedia's test shot recommendation for kickstaring things, I didn't put a lot of stock in the type of specialist he may have recommended.
