Since it has been the time it's been, 4-5 months... Are my balls at permanent damage and will nolva/HCG even still be useful at this stage. I've studied the Nolva and have experience taking it, I'm not too familiar with HCG.

There is no way of knowing if any damage is done or if it´s permanent. Shrunk balls does not mean damage.
Yes Nolva/HCG will be useful even after 5 months, but not if you are experiencing Pituitary failure.
Go get BW, so you don´t have to guess. You should be doing BW anyway before and after a cycle.

How long after the cycle should bloodwork be done?

I do BW about once/month or whenever I feel different. If you can only do BW once, I´ve seen 1 month post PCT been said, but I would say 2 months is a better gauge. Homeostasis can take much longer, everybody is different.
