    When you squat your knees shouldn't go over (forward or past) your knees. It damages them.... The meniscus, ligaments, cartilage.

    I have to look into the width because I always keep my feet at shoulder width or just beyond.

    Ya I've never went past being parallel with shoulder placement in backsquats. In front squats I have to exaggerate the spacing and increase the degree of lateral rotation to my knees. I don't know if this is cheating, but on front squats I put something underneath my heels to help with maintaining a neutral spine. I find it really easy to fall forward during the eccentric portion.

    With squat or deadlift, I also raise my toes off of the in-sole of my shoes. This distributes the weight properly throughout my feet keeps me from rolling forward. I learned that in yoga but it actually translates very well into squat-type movements.
